Root canal treatment (RCT) is done to repair decayed or infected teeth and save them from extraction. The nerves and bacteria are cleaned up and removed. The pain is gone and tooth is saved. High success rates are seen with RCT(86-98%) with proper maintenance. But sometimes, teeth don’t heal properly and can fail (2-14% ).

These are the reasons for RCT failure:

  • Narrow or curved root canals
  • Complicated canal anatomy with obstructions
  • Crown not placed
  • Salivary contamination
  • New decay or infection around crown margins

How is it diagnosed?

Based on symptoms and radiographic features, it is diagnosed and treated via any of these 3 additional procedures

  • 1) Re-RCT: The teeth will be re-cleaned and re-filled. It is usually the first option if failure occurs. If this option is not feasible, then apical surgery is done.
  • 2) Apical surgery or root end surgery: It’s suggested in cases where re-treatment is not possible or successful or in addition to re-RCT. Here the root end is surgically accessed and the infection is removed at the root tip and sutures are placed.
  • 3) Extraction or tooth removal: It’s the final step when the teeth is not responding to any treatment. The teeth is removed and a replacement (dental implant or bridge or denture) needs to be done later after the healing period to restore function and esthetics.
